Cirrosis hepática: de la etiología al tratamiento integral

La cirrosis hepática constituye la etapa final común de múltiples agresiones crónicas al hígado y representa una importante causa de morbilidad y mortalidad a nivel global. Esta revisión narrativa tiene como objetivo sintetizar la evidencia actual sobre su etiología, fisiopatología, diagnóstico, complicaciones, manejo y pronóstico. La cirrosis se caracteriza por la sustitución del tejido hepático normal por nódulos y fibrosis, lo que conduce a una pérdida progresiva de la función hepática. Las causas más frecuentes incluyen el consumo de alcohol, las infecciones por los virus de la hepatitis B y C, las enfermedades autoinmunes y, cada vez más, la disfunción hepática grasa asociada al metabolismo (MASLD). Clínicamente, la enfermedad puede permanecer silente hasta su descompensación, manifestándose con ictericia, ascitis, encefalopatía o hemorragia digestiva. El diagnóstico se basa en la combinación de hallazgos clínicos, pruebas bioquímicas y métodos no invasivos, como la elastografía. El tratamiento es integral e incluye el manejo etiológico, la prevención de complicaciones y, en casos avanzados, el trasplante hepático. Las escalas Child-Pugh y MELD permiten estimar la gravedad y el pronóstico de la enfermedad hepática crónica; en particular, el modelo MELD se utiliza para priorizar a los pacientes en lista de trasplante hepático. El abordaje temprano y multidisciplinario puede contribuir a mejorar la supervivencia y la calidad de vida, especialmente en etapas iniciales donde algunas formas de enfermedad hepática pueden ser potencialmente reversibles.
